What this form is not for

For these, the resources below answer faster and more accurately than we can.

Flight training enrollment

We don't broker training. Contact a 14 CFR Part 61 instructor or a Part 141 helicopter school directly. Bristow Academy (FL), Hillsboro Aero Academy (OR), Quantum Helicopters (AZ), Mauna Loa Helicopters (HI), and Upper Limit Aviation (UT) are common starting points.

FAA medical questions

Medical special issuances, deferrals, BasicMed eligibility, and aircraft-specific medical fitness route to your Aviation Medical Examiner (AME). FAA's AME locator: faa.gov/pilots/amelocator. For appeals, the FAA Office of Aerospace Medicine handles cases.

Legal interpretation of 14 CFR

Specific interpretations of FAA enforcement actions, certificate suspensions, accident-related certificate review, or complex multi-rule scenarios need an aviation attorney. AOPA Pilot Protection Services or NBAA legal staff are good starting points for referrals.

Accident reporting

If you witnessed or were involved in a rotorcraft accident or incident, report directly to the NTSB (notification under 49 CFR 830 within hours for accidents) and the FAA Flight Standards District Office (FSDO). This site does not collect accident reports.
